Workshop Registration Form

WORKSHOP REGISTRATION FORM

This registration form is provided for students who do not wish to pay for workshops using our online payment system. Use this form to register online and then pay by cash or check. To pay online, please go back to the workshop page and choose a payment option where you will also be able to register.

Your place in a workshop can only be held upon receipt of deposit + registration fee, or full tuition + registration fee, made by check or cash. Registration fee is non-refundable.
Please complete the registration form below to temporarily hold your place. Your place can be held for up to 5 working days before payment is received. We will contact you by phone if your payment is not received within the aforementioned time frame.

For students under 18, please include parent or guardian name.

TERMS and CONDITIONS: Submission of this Registration Form constitutes my acceptance of all terms and conditions stated in this contract and agreement that the Oklahoma Academy of Classical Art (OACA) cannot be held responsible or liable to me for any act or for any injury, illness, death, damage, loss, accident, delay or irregularity which may occur during the course of any program. Personal belongings, art and art supplies are solely my responsibility at all times. OACA reserves the right to cancel or reschedule any class in which case it is liable only for the tuition I paid to the OACA. OACA reserves the right to cancel my enrollment in the event of any disruptive or unlawful behavior without refund to me. I understand I am obliged to abide by the payment terms listed in this document. Unless otherwise informed, OACA considers photographs taken during classes and workshops of students and student art permissible for publication in print and on the web.
