This registration form is provided for students who do not wish to pay for courses or workshops using our online payment system. To pay online, please go back to the workshop page and choose a payment option where you will also be able to register.

Your place in a workshop can only be held upon receipt of deposit or full tuition made by check or cash.
Please complete the registration form below to temporarily hold your place. Your place can be held for up to 5 working days before payment is received. We will contact you by phone if your payment is not received within the aforementioned time frame.

For students under 18, please include parent or guardian name.

TERMS and CONDITIONS: Submission of this Registration Form constitutes my acceptance of all terms and conditions stated in this contract and agreement that the Oklahoma Academy of Classical Art (OACA) cannot be held responsible or liable to me for any act or for any injury, illness, death, damage, loss, accident, delay or irregularity which may occur during the course of any program. Personal belongings, art and art supplies are solely my responsibility at all times. OACA reserves the right to cancel or reschedule any class in which case it is liable only for the tuition I paid to the OACA. OACA reserves the right to cancel my enrollment in the event of any disruptive or unlawful behavior without refund to me. I understand I am obliged to abide by the payment terms listed in this document. Unless otherwise informed, OACA considers photographs taken during classes and workshops of students and student art permissible for publication in print and on the web.

A non-refundable registration fee is required for each new student. If you are currently enrolled in CCA classes, the registration fee does not apply. Simply fill out the registration form for each enrollment. If you are enrolling more than one student, you may use the same form. Thank you!

Thank you for your enrollment! If you are a new student or have been absent from CCA classes for 3 or more months, please pay the $25 non-refundable registration fee by clicking on the button above, or mail to: CCA, 12 E. Burton Place, Edmond, OK 73013.
